Payment 

Evaluations last roughly 60-90 minutes, and will consist of:

• Individualized formal testing and interview
• Unique and personalized goal development
• Treatment planning

We accept the following insurance providers:​

If we are not in-network with your provider, you will be provided a superbill, which then can be submitted to your insurance company for reimbursement. Channels Rehabilitation LLC does NOT guarantee reimbursement from insurance companies. Payments will be rendered when services are provided.
